Confirmation of Injuries

A medical certificate is required from the attending medical doctor stating what the injuries were and the duration of the victim’s incapacity

Confirmation of Death

This requires a Certificate of Cause of Death from the doctor who attended to the victim or a Death Certificate from the Ministry of Justice and Court Administration.

Confirmation from the employer

The employer is required to confirm and send a report to ACC bout the worker’s accident within 5 days of the accident.

Confirmation from Police

The Ministry of Police is required to report to ACC a motor vehicle accident or sea-going vessel accident within 5 days of the accident.

Other documents may include:

Marriage certificate of parents of the deceased
Birth certificate of claimant where the claimant is a dependant of the deceased.
